A patient came to see me recently convinced she was in perimenopause.

Weight gain. Chronic fatigue. Depression. Anxiety. All of it. And honestly, on the surface, it made sense. She was in her late 40s and all of the symptoms fit. But something made me slow down and ask a few more questions.

She had taken a new job as a middle school teacher. She was working late evenings and weekends. She was eating well but had stopped working out because she said she had no time. She had stopped meeting up with her friends because, once again, she had no time. And every night, without fail, she was falling into bed well after midnight and waking up unrefreshed, already behind before the day had begun.

I looked at her and said what I say to every single patient who comes into my office: “The most important thing we need to fix is your sleep.”

She looked back at me like I had said a bad word.

She wanted a full hormone panel because she was convinced her symptoms pointed to more. She wanted what I call ‘a big answer.’ And I understood that completely. None of us wants to miss something real. But what I have learned in over thirty years of clinical practice is this: sleep is not the soft answer. It’s the most powerful, most underutilized, most criminally underestimated prescription in medicine. And until we address it, nothing else we do will work the way it should.

The Glymphatic System and Why Your Brain Desperately Needs Sleep 

Let me tell you something that most patients (and frankly many clinicians) do not fully appreciate: your brain does not rest while you sleep. It actually cleans itself.

During the deepest stages of sleep your brain activates a remarkable biological sanitation system called the glymphatic system — a network of channels that surrounds your brain’s blood vessels and acts as a lymphatic drainage system for the central nervous system.

While you are awake this system is largely dormant. But the moment you enter deep, slow-wave sleep it opens fully. Cerebrospinal fluid begins flowing rapidly through the brain, flushing out metabolic waste products that have accumulated throughout the day.

And the most important waste product it clears? Beta-amyloid and tau protein — the same toxic proteins that accumulate in the brains of people with Alzheimer’s disease.

Let that land for a moment.

Every night of insufficient sleep is a night your brain’s cleaning cycle runs short. And every night it runs short, a little more waste is left behind. Not dramatically. Not visibly. Just quietly, incrementally, the way a drain that’s never fully cleared eventually clogs.

This isn’t a metaphor. This is neuroscience. And it is one of the most compelling reasons I have ever encountered to take sleep with the same seriousness we take diet, exercise, and medication.

Sleep architecture — what’s actually happening while you sleep

 

Sleep is not a single state. It is a precisely orchestrated series of stages that cycle through the night in roughly 90-minute intervals, each serving a distinct biological purpose.

Stage 1 and Stage 2 — Light Sleep The transition from wakefulness to sleep. Your body temperature drops. Your feart rate slows. The brain begins to disengage from the external world. This is where most people spend far more time than they realize when sleep is fragmented — cycling in and out of light sleep without ever reaching the deeper stages where the real restoration happens.

Stage 3 — Deep Slow-Wave Sleep This is where the glymphatic system activates fully. It’s where growth hormone is released; the hormone responsible for cellular repair, muscle recovery, and immune function. It’s where the hippocampus consolidates the memories of the day and transfers them to long-term storage. It’s where blood pressure drops to its lowest point and the heart gets its most complete rest. Deep sleep is the most biologically restorative stage of the cycle and it is the first stage we lose when sleep is chronically insufficient or fragmented.

REM Sleep — Rapid Eye Movement This is where the brain processes emotion, integrates experience, and does the creative, associative work that makes us capable of insight, empathy, and complex problem-solving. REM sleep is also where the brain regulates the amygdala — the brain’s alarm system. When REM is cut short or suppressed — by alcohol, by late-night screens, by stress hormones — the amygdala becomes hyperreactive. You wake up already anxious, already irritable, already braced for threat, not because anything has gone wrong in your life but because your brain’s emotional regulation system did not get the maintenance it needed overnight.

My patient who came in convinced she was in menopause? Her anxiety and low mood were not hormonal. They were neurological. They were the predictable, physiological result of a brain running chronically short on REM.

The hormones your sleep is regulating whether you know it or not

 

Back to my patient and the weight gain she couldn’t explain.

She was eating well. She was doing everything right during the day. And still the scale kept moving in the wrong direction. This is one of the most demoralizing experiences a patient can have and it is also one of the most clinically explainable when you understand what sleep does to the hormones that regulate hunger and metabolism.

 

Ghrelin and Leptin — the hunger hormones

 

Leptin is the hormone that signals satiety — that tells your brain you have had enough, stop eating. Ghrelin is the hormone that signals hunger, telling your brain you are hungry, find food. Both are regulated during sleep.

When sleep is chronically insufficient leptin drops and ghrelin rises. The result is predictable and relentless. You feel hungrier than your body actually needs you to be, and the fullness signal that should tell you to stop eating arrives late or not at all.

Studies consistently show that sleep-deprived individuals consume significantly more calories (particularly from high-fat, high-carbohydrate foods) than their well-rested counterparts. Not because they lack discipline but because their hormones have been dysregulated by an inadequate night of sleep.

 

Cortisol — the stress hormone

Insufficient sleep also elevates cortisol, the primary stress hormone. Elevated cortisol promotes visceral fat storage particularly around the abdomen. It promotes insulin resistance, raises blood pressure, and suppresses the immune system. It also creates a physiological state of chronic low-grade stress that makes everything harder — mood, focus, energy, resilience — regardless of how well you manage your external circumstances.

My patient didn’t have a hormone problem. She had a sleep problem that was creating a hormone problem (big difference!) The distinction matters enormously because the treatment is completely different.

 

What the Blue Zone communities know about sleep that we have forgotten

 

The world’s longest-lived populations — in Okinawa, Sardinia, Ikaria, Nicoya, and Loma Linda — share something that longevity researchers have only recently begun to appreciate fully: it’s not just what they eat or how they move. It’s how they rest.

In Ikaria — the Greek island where people routinely live past ninety — afternoon napping is not a luxury. It’s a cultural norm embedded in the daily rhythm of life. Research published in the Archives of Internal Medicine found that regular napping was associated with a 37% reduction in coronary mortality — a finding so significant that the researchers described the Ikarian relationship with rest as a major contributing factor to the island’s extraordinary longevity.

In Okinawa the concept of ikigai, which literally meansa reason for being, is deeply intertwined with the quality of rest.

The Okinawans do not hustle. They do not glorify busyness. They structure their days around meaningful activity followed by genuine rest — not the collapsed exhaustion of someone who has given everything and has nothing left, but the chosen stillness of someone who understands that restoration is itself a form of living well.

We have built a culture that treats sleep as negotiable; as the thing you sacrifice when there are more important things to do.

The Blue Zone communities have built cultures that treat sleep as sacred; as non-negotiable as eating, as essential as breathing.

Plant Powered Medicine: Passionflower

Before we talk about what to buy, let me tell you what to make.

There is something deeply intentional about brewing a cup of tea before bed. The ritual of it — the kettle, the warmth, the slowing down — is itself a signal to your nervous system that the day is closing. Add passionflower to that ritual and you have something that is both ancient and clinically supported.

Passiflora incarnata (also known as passionflower) has been used as a calming botanical for centuries across Native American, European, and Ayurvedic traditions. Modern research has confirmed what those traditions understood intuitively: passionflower works through GABA modulation — the same neurotransmitter pathway that pharmaceutical sleep aids target, but gently, without the dependency or the morning fog.

A randomized controlled trial published in Phytotherapy Research found that passionflower tea significantly improved subjective sleep quality compared to placebo. Participants reported falling asleep more easily, waking less frequently, and feeling more rested in the morning. The mechanism is elegant.

Passionflower increases GABA activity in the brain, quieting the nervous system’s background noise and allowing the natural sleep pressure that has been building all day to finally do its work.

Simple Passionflower Sleep Tea

A five-minute ritual worth keeping.

 

You will need:

1 heaping teaspoon dried passionflower herb (organic is best)

8 ounces of water just off the boil — not a full rolling boil, which destroys the delicate volatile compounds

A ceramic mug you love

Honey or a small slice of fresh lemon (optional but lovely)

The ritual:

Place the passionflower in a small strainer or tea infuser.

Pour the hot water over it.

Cover the cup (a small saucer works perfectly) and steep for eight to ten minutes. The covering matters: the volatile compounds that contribute to passionflower’s calming effect are released as steam and you want them in your cup, not evaporating into the air.

Remove the infuser. Add honey or ginger if you like.

Sit with it somewhere quiet. Let it be the last warm thing you do before sleep.

Begin this ritual at least forty-five minutes before your intended sleep time. Pair it with dimmed lights and no screens. Your nervous system will learn — the way all rituals teach — that this sequence means the day is over and rest is now the priority.

06 · FIVE THINGS YOU CAN DO TONIGHT

Before we close — because clinical information without clinical application is just another interesting thing that gets filed away — here are five evidence-based sleep interventions you can begin tonight:

1. Set a non-negotiable lights-out time. Not a bedtime goal. A commitment. The research on sleep regularity — going to bed and waking at the same time every day including weekends — shows it is as important as duration for cognitive function and metabolic health.

2. Stop eating two to three hours before bed. Late eating elevates insulin and body temperature both of which interfere with sleep onset and the deep sleep stages. Your digestive system needs to be winding down before your brain can fully enter restoration mode.

3. Lower your bedroom temperature. Core body temperature must drop by approximately one to two degrees Fahrenheit to initiate and maintain deep sleep. The optimal sleep temperature is between 65 and 68 degrees Fahrenheit. This is not a preference — it is physiology.

4. Eliminate screens one hour before bed. Blue light from screens suppresses melatonin — the hormone that signals darkness and prepares the brain for sleep. But the content of what we consume is equally disruptive — news, social media, and email activate the amygdala and raise cortisol at precisely the moment we need both to be declining.

5. Create an exit ritual from the day. This does not need to be elaborate. A specific sequence of small actions — dimming the lights, making herbal tea, writing three things you are grateful for, a short breathing practice — that signals to your nervous system that the day is complete and rest is now the priority. The ritual itself becomes a neural cue over time. Your body begins to prepare for sleep before your head hits the pillow.

 

07 · UNTIL NEXT TIME

 

Remember my patient, the teacher who came in convinced she was in perimenopause? She came back to see me three weeks after we addressed her sleep.

She set a firm lights-out time. She stopped working past nine o’clock. She reclaimed her weekend mornings. She hadn’t changed her diet or restarted her workouts yet because we decided to address sleep first and only sleep.

Progress: She lost four pounds. Her mood stabilized (no more mood swings!) and her energy returned. She looked like a different person.

She was not in perimenopause. She was sleep-deprived. And her body — given just three weeks of adequate rest — had begun to remember how to regulate itself.
